G'day, Yay Team ! That's pretty scary. I wonder...., back in the 1980s when Composites were actually a bit new, there was a spate of Glassfibre Sailplanes Spontaneously disintegrating in fine weather and calm conditions...; and it turned out that they had all, on some previous flight or flights, climbed sufficiently high as to have become cold enough that the minute Air-Inclusions in the Composite Matrix which were built in during manufacture, first chilled to the point that the Water from the Humidity in the Air Inclusion condensed & coalesced into a Droplet... And then the Droplet/s froze, and EXPANDED as they became Ice Crystals, and the expanding of the growing Crystals fractured the Resin, delaminating the layup ; so that normal cyclic Flight-Loads stressed & gradually extended the Cracks. And, then, one bright day the Sailplane suddenly became Fibreglass Confetti in the Sky. The price of Fibreglass Sailplanes which had been above 15,000 ft fell to almost nothing - until the manufacturers learned to Vaccumn-Bag their Layups and then Oven-Cure the resulting hardened Components... In about 1985 an Altitude Record-Holding Ultralight in Australia, which held the Record at about 35,000 ft, 6 months or so earlier, fell to buts on a Sunday Afternoon while on a Cross-Countrynat 2,000 ft....; same story..., the Mitchell U-2 Flying Wing featured a Fibreglass Wingspar - and it had been built at the Coast, in Hot Humid, Seaspray-laden Air.... One wonders quite what Precautions the Pasta-Eaters back in Italia may have perhaps neglected to fully observe...; when they were laying up the Fuselage, Tail, & Wings....? If they failed to Vac.Bag it - and merely laid it out and Hand-Squeegie-ed away the excess Resin... (possibly during the CoViD/post-CoViD chaos & confusion...(?) - then that might explain a lot... Perhaps. It was my first-cut Provisional Diagnosis, so it's barely better than a wild guess ; but it IS a known mechanism for causing Composite Hairygoplanes to spontaneously delaminate and fall to Earth in multiple locations.... Which appears to be EXACTLY what this thing did. And, by the way, anyone who thinks this is any kind of "upgraded Ultralight" has not much of a clue as to what an Ultralight actually IS. Australian Air Navigation Order 95.10 was Gazetted in August 1976, written around, "The 8-hp, 1975, Red Baron Skycraft Scout ; World's 1st Minimum Aircraft...!" search YT for that, to see the Video Slideshow, for a more recent video try, "National Transportation Museum ; Visiting My First Aeroplane...!". ANO-95.10 stipulated Max. T.O.-weight of 400 Pounds, not to go above 300 ft from Terrain, not within 100 ft vertically or 500 ft horizontally of any Cloud, not to cross Paved Roads, not to operate or fly within 5 miles of any Airport or Controlled Airspace, not to overfly any Race Meeting or Regatta or Public Gathering, not to overfly Municipal Council Areas....; Inside those limits, ALL OTHER ANO's DID NOT APPLY...., no C. of A., no Licence, no Inspections, just HAVE Fun. Ultralights could be, under ammended Orders, 480 Lb for Single Seaters & 630 for 2-Seaters. Gotta go, I hope that helps. Stay safe... ;-p Ciao ! ANO

What emergency procedure requires the pilot(s) to first request a lower flight level upon a declared emergency?

Typical Boeing quality control. People need to sue them for this. This is NOT ok.

why did the pilots of JAL516 fail to see the dash 8 on the runway on final on a clear night? they must have had 30-40 seconds to see the runway incursion and execute a go-around.
